Additionally, a 404 Not Found error was encountered while trying to use an ErrorDocument to handle the request.

Thanks for the heads-up. It's fixed now. I think it had to do with the fact that the title had ".kmz.htm" at the end - the server got confused. I renamed it and it's coming up fine now:
