It's a neat find! :)

However, we won't really have a good idea if it's in trouble or not I think.

They do apparently often sleep on their sides. I'd read in the past about their sleep patterns and there were a lot of pictures of healthy horses who were sleeping on their sides. However, when a horse is very sick or dying/dead, they obviously also do lie on their side like that too. Being a snapshot, it's also difficult to say for sure if the horse is just rolling around too.

One suggests that sleeping on a side is a sign of the horse being older:

It's a sleeping horse.

If it was dead the head would be laying more behind to gasp aster air in their final breath. Just the same as with humans.

If you look at the image the head is laying in the same position as standing up. A normal position for horses. :)
